OFFICIAL HIGH SCHOOL OR GED TRANSCRIPT

If you have graduated high school or passed a high school equivalency test (ex. GED) please electronically send us your transcripts. These can be sent through Parchment or emailed to admissions@cmn.edu.

Please allow two weeks to six weeks if your transcript is mailed.

Testing Scores (ACT, SAT, etc.)

If you have taken a placement test such as the ACT or SAT, please electronically send us a copy of your test scores. These can be emailed to admissions@cmn.edu. Please note that there may be a delay in service if your test scores are mailed.

College Transcripts

Official college transcripts can be sent to admissions@cmn.edu. We can receive electronic transcripts from the National Student Clearinghouse and Parchment.  Transcripts sent via postal mail may see delays in processing times.

IDENTIFICATION

Driver’s License, State ID, Birth Certificate, or Social Security Card

Tribal Citizenship

Proof of membership of a federally recognized tribe.

Vaccination Records

Oklahoma state law requires that all new students attending Oklahoma colleges and universities for the first time provide proof of immunization for:

  • Three Hepatitis B
  • Two Measles-Mumps-Rubella (MMR)
  • Meningitis (housing residents only)

These can be obtained through your High School or your local Health Department. Exemptions can be obtained through the Admissions Office.
