OFFICIAL HIGH SCHOOL OR GED TRANSCRIPT

All transcripts must be official from the issuing school and sent directly to the Admissions Office. To ensure the transcripts are official, students must have the issuing school send the electronic transcript directly to admissions@cmn.edu or by mail to CMN 2170 Raven Circle Okmulgee, OK. 74447.

Please allow two weeks to six weeks if your transcript is mailed.

Testing Scores (ACT, SAT, etc.)

If you have taken a placement test such as the ACT or SAT, please electronically send us an official copy of your test scores. Official test scores can be emailed to admissions@cmn.edu. Please be aware that processing your service may take longer if your test scores are sent by mailed.

Once you have submitted your test scores, an admissions representative will notify you of additional testing.

College Transcripts

All college transcripts must be official from the issuing institution and sent directly to the Admissions Office. To ensure the transcripts are official, students must have the issuing institution send the electronic transcript directly from the National Student Clearinghouse and/or Parchment. Please send official transcripts from all institutions attended as a student.  Electronic transcripts can be sent to admissions@cmn.edu or mailed to CMN 2170 Raven Circle Okmulgee, OK. 74447.

Transcripts sent via postal mail may see delays in processing times.

IDENTIFICATION

Driver’s License, State ID, Birth Certificate, or Social Security Card

Tribal Verification

Proof of membership of a federally recognized tribe.

Vaccination Records

Oklahoma state law requires that all new students attending Oklahoma colleges and universities for the first time provide proof of immunization for:

  • Three Hepatitis B
  • Two Measles-Mumps-Rubella (MMR)
  • Meningitis (housing residents only)

These can be obtained through your High School or your local Health Department. Exemptions can be obtained through the Admissions Office.
